Qualifications:
What You Will Do
As an RN Case Manager, you will play a vital, hands-on role in providing compassionate hospice care, including:
- Performing physical and psychosocial assessments
- Identifying hospice diagnoses and documenting ongoing eligibility
- Delivering skilled nursing care
- Assessing and managing pain and symptoms
- Ensuring the highest level of comfort and quality of life
- Educating patients and families on end-of-life processes and rights
- Developing and coordinating individualized plans of care
- Initiating referrals and providing ongoing emotional and palliative support
- Supervising hospice aides per Medicare requirements
Previous hospice experience strongly preferred.
What We’re Looking For
- Registered Nurse (RN) licensed in Pennsylvania, in good standing
- 1 year of recent hospice experience preferred
- Strong communication, customer service, and critical thinking skills
- Ability and willingness to travel within the service area
- Valid PA driver’s license, personal vehicle, and insurance
- Ability to lift/move 50 lbs and transfer patient weight of 100 lbs
- Ability to perform physical job functions (walking, bending, standing, etc.)
