Bridges Healthcare, Inc. has an immediate opening for a Tobacco Treatment Specialist. 

$24.87 - $30.48 per hour, commensurate of experience.

PURPOSE and POSITION OVERVIEW

The Tobacco Treatment Specialist provides evidence-based tobacco cessation services to individuals and groups while supporting broader system and community efforts to reduce tobacco use. This role delivers counseling, conducts screenings, and facilitates referrals to cessation resources, including the Connecticut Quitline, while collaborating with healthcare providers and community partners to integrate tobacco treatment into clinical practice. The position also supports outreach, training, program implementation, and data collection to ensure compliance with Department of Public Health (DPH) requirements and achievement of program outcomes.

 

DUTIES and RESPONSIBILITIES

  • Provide individual and group tobacco cessation counseling using evidence-based models (LAHL for adults, NOT for youth).
  • Develop quit plans, including relapse prevention and follow-up support.
  • Conduct follow-ups to monitor progress and provide ongoing support.
  • Facilitate referrals to CT Quitline and other cessation resources.
  • Support program work plan activities, including outreach and service delivery goals.
  • Collect and document service data and assist with required reporting.
  • Ensure compliance with DPH documentation and reporting requirements.
  • Conduct tobacco screenings and implement evidence-based approaches (e.g., 5As, SBIRT).
  • Integrate tobacco treatment into clinical workflows in coordination with providers.
  • Deliver community education and outreach presentations.
  • Distribute educational materials and participate in community outreach activities.
  • Provide training and technical assistance to staff and partners.
  • Collaborate with internal teams, community partners, and coalitions.
  • Maintain required certifications and adhere to best practices.
  • Participate in trainings and meetings.
  • Meet enrollment targets and service delivery goals as defined by program benchmarks.
  • Maintain timely and accurate documentation and reporting.
  • Participate in quarterly reporting and outcome tracking processes.
  • Perform other duties as assigned.

 

QUALIFICATIONS AND COMPETENCIES

Bachelor's degree in behavioral health, public health, social work, or related field required; relevant experience may be considered in lieu of degree.

Experience providing individual and/or group services in a behavioral health or healthcare setting.

Experience with health education, counseling, or prevention programming preferred.

Ability to work collaboratively with healthcare providers and community partners.

Strong documentation and organizational skills; proficiency with electronic health records and Microsoft Office (Word, Outlook).

Valid Driver's License with access to reliable transportation on a consistent basis.
