Harry Hynes Memorial Hospice was founded by community volunteers in 1983 as a nonprofit organization, we are honored to be a leading provider of Hospice and Palliative care in South Central and Southeast Kansas. Having served more than 40,000 patients and family members, we are experts in specialized care designed to provide support to you and your loved ones during an advanced illness. Our team of professionals, including a medical director, nurses, hospice aides, social workers, and volunteers, work in coordination with the patient’s physician to provide pain and symptom management, as well as social and spiritual support.

Summary

The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes, assessing patient needs, planning and implementing care, coordinating with the care team, and educating patients and their families. The RN ensures quality care while meeting federal, state, and agency standards.

Essential Functions

· Complete an initial nursing evaluation within 48 hours of referral, per Kansas regulations.

· Conduct ongoing assessments of patient condition (physical, emotional, social) during home visits.

· Develop and update individualized plan of care in collaboration with physicians, other nurses, and interdisciplinary team.

· Perform skilled nursing duties: wound care, IV therapy, catheter care, medication administration, chronic disease management.

· Provide direct nursing interventions based on patient needs and plan of care.

· Monitor and document patient response to treatments, including vital signs, lab results, and symptoms.

· Teach patients and caregivers about disease processes, medication management, self-care, safety, and home health best practices.

· Provide guidance on health maintenance, fall prevention, nutrition, and equipment use.

· Serve as a liaison with physicians, physical/occupational therapists, social workers, and other healthcare providers.

· Coordinate scheduling and execution of visits.

· Update and revise care plans as patient condition changes.

· Maintain accurate and timely documentation in the agency’s electronic medical record (EMR), including OASIS (if applicable).

· Report changes in patient condition to supervising nurse or physician immediately.

· Ensure care meets Medicare, Medicaid, and state regulatory requirements.

· Follow agency policies, procedures, and standards of care.

· Provide oversight, clinical direction, and supervisory visits/checks to Licensed Practical Nurses (LPNs), home health aides, and other support staff as appropriate.

· Participate in quality improvement efforts, in-service training, and continuing education.

· Maintain required certifications and licensure.

· Travel to patient homes within assigned geographic territory.

· Provide competent care while navigating a variety of home environments.

· Other duties will include, but are not limited to, visits to Bridges HealthCare palliative patients and nursing visits for Harry Hynes Memorial Hospice if necessary.

Requirements for nursing skills across all departments of Hospice, Inc. and subsidiaries

1. The registered nurse is responsible for providing nursing care to patients and families utilizing the nursing process.

· A thorough assessment, covering all aspects of the patient/family, including physical, emotional spiritual, social, psychological and cultural status should be obtained and documented appropriately.

· Critical thinking skills should be applied to the analysis of the data collected.

· Opportunities for improvement in palliation of symptoms should be identified and appropriate nursing diagnoses documented.

· A plan of care is developed with interventions that adhere to the standards of hospice nursing.

· The plan of care is developed with input from the patient/family and the interdisciplinary team.

· The registered nurse is responsible for the communication and implementation of the plan of care in all settings.

· Ongoing evaluation of progress toward appropriate outcomes is done and documented.

· Reassessment and revision of the plan of care is completed as needed in collaboration with patient, family, and interdisciplinary team and documented.

2. The registered nurse is required to maintain current knowledge and competency of hospice, home health, and palliative nursing.

3. The registered nurse contributes to an environment conducive to educating and welcoming nursing students, other healthcare students, and other members of the interdisciplinary team, as appropriate.

Qualifications, Education and Experience

· Successful completion of a Registered Nurse program through an accredited institution.

· BSN strongly preferred.

· One (1) year of medical/surgical experience with Hospice, Oncology, Home Health/Private Duty experience being preferred.

· Basic computer skills.

· Knowledge of acceptable RN nursing practices and procedures.

· Demonstrated knowledge of universal precautions.

· Demonstrated ability to communicate effectively.

· Current licensure as an RN in the State of Kansas.

· Valid drivers’ license.

· Proof of current auto insurance.
