ORGANIZATION DESCRIPTION
Bridge Meadows is an intentional intergenerational community that supports permanence for children impacted by child welfare, supports their parents to thrive, and provides opportunities for meaning and purpose for older adults (elders). Parents, youth, and elders receive professional and natural support from staff, collaborating agencies, and one another, moving them toward the achievement of success in academics, health and well-being, and housing stability.
Summary
The Redmond Community Support Manager (CSM) is responsible for leading the Bridge Meadows programming on-site with the community, leading to outcomes of housing stability, safety, connection, and belonging. The CSM plays a key role in supporting residents' well-being and housing stability by combining community resource coordination with professional therapeutic practices. This position focuses on fostering a safe, inclusive, and supportive living environment through direct clinical service, advocacy, community-building, and programming.
The CSM role is the heart of Bridge Meadows and facilitates the building of healthy, intentional, intergenerational living. In addition, the position catalyzes and supports community-wide events that foster intergenerational connection and community support for residents, living into the program value of doing things “with and not for” those we serve, the CSM assists individuals with accessing resources that lead to greater thriving, coordinates with local-level partners, and implements programmatic elements into the Bridge Meadows community. The foundation of the intergenerational model relies on the community supporting one another, and the CSM has clinical experience in understanding trauma, conflict-resolution, racial equity and belonging, mental health issues, and addressing relationship building.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Clinical Work
- Facilitate and support the strengthening of community members with psychosocial/educational activities.
- Provide case management services, including goal setting, referrals, community resources, follow-up, and documentation.
- Assist residents in navigating personal crises involving domestic violence, substance abuse, mental health, eviction risk, or medical emergencies, providing resources, plans, and referrals for ongoing support.
- Maintain an active presence in the community through regular outreach, visits, and community engagement.
- Provide referrals to service providers and partners and when needed, serve as liaison between community members and providers/partners to monitor appropriateness of service delivery
- Utilize clinical supervision to problem-solve community member issues and identify areas of professional growth
- When appropriate, establish and maintain linkages between community members, family members, and other community agencies to address needs or issues
- Guide, advise, train, and/or facilitate community members in problem-solving and conflict resolution
Program Leadership
- Facilitate small groups and community gatherings that help to meet the needs of the community to accomplish programmatic outcomes (housing stability, safety, connection, and support)
- In collaboration with community members, design and coordinate programs that meet collectively identified needs
- Coordinate with external partners to help support programmatic elements within the community
- Work directly with community members to support intergenerational and social connection and engagement
- Participate in 1-1 meetings with supervisor and program team to identify community successes, challenges, opportunities, and innovation
- Manage program support staff, if applicable
- Lead community-wide communication strategy and execution
Data and Compliance
- Utilize the Bridge Meadows data management system to track community members’ progress, community participation, and gaps in program services
- Administer and manage records like intake paperwork, emergency contact, and outcome survey data for community members
- Prepare narrative and programmatic reports on service outcomes. This is generally coupled with data to tell the story of Bridge Meadows to funders, supporters, and evaluators
- Participate in audits and program evaluations
Collaboration
- Actively participate with the Bridge Meadows’ team on projects and organizational activities
- Collaborate with the site’s property management in addressing and intervening in community member issues and crises
- Advocate for community members’ needs and provide social and emotional support to individuals amidst housing-related issues
- Collaborate with Bridge Meadows’ staff around complex issues for individuals at the intersection of housing and clinical work
- Additional responsibilities, as assigned
REQUIREMENTS
- This is an in-office position at the Bridge Meadows’ site with occasional travel to community partners or other Bridge Meadows’ locations
- Supervisor approves requests for work-from-home for administrative tasks
- Occasional evening or weekend work is required for community events based on our residents’ availability, scheduled outside of the typical (9am-5pm) workday. This role flexes time when evening or weekend work is required.
- This position may receive calls or texts after hours regarding emergencies such as death, medical, or safety issues that impact the whole community. Therapeutic support is not required after hours.

- Housing stability is prioritized for each community member, and includes supporting community members with psychosocial support and tangible resources to lead to housing stability
- Connection is facilitated with and between community members
- Clinical support is provided 1-1 and in group sessions to lead to a greater sense of individual belonging and community flourishing
QUALIFICATIONS
Knowledge of:
- Clinical understanding of mental health issues, human behavior, child development, older adult issues, trauma, systems and group dynamics
- Challenges of families involved in the child welfare system and social support required
- The aging process
- Belonging and the role that racial cultural context impacts community living, and offering programs in a culturally responsive manner
- Housing issues and resources for families and older adults
- Assessment and diagnostic skills
- Microsoft Office and general office management
Ability to:
- Exemplify traits that reflect Bridge Meadows’ culture and values
- Use creativity, critical thinking, and empathy to expand and evolve intergenerational living
- Exercise excellent judgment
- Coordinate day-to-day community activities, in collaboration with the property manager, community partners, and Bridge Meadows team as necessary
- Build collaboration and a sense of agency among community members
- To cooperate as a member of an interdisciplinary treatment team
- Establish clear and appropriate boundaries in relationships with staff and residents and the handling of confidential and sensitive information
- Initiate and work independently
- Gain an understanding of Fair Housing Laws
Education: Master’s degree in counseling, social work, psychology, or a related field or equivalent experience.
Certificates, Licenses, Registrations: Maintain a driving record acceptable by the agency and a current driver’s license for state of residence. When authorized use own vehicle for agency business; employee must maintain personal insurance.
Experience: Minimum of five years of practical experience in direct client care and/or counseling.
