SUMMARY
At the direction of the Licensed Administrator, the Social Worker will plan, organize, develop, direct, and evaluate the resident social work function in accordance with current applicable Federal, State and local standards. The Social Worker will coordinate discharge planning, assist patients and their families to achieve and maintain maximum functioning in and outside of the community through needs assessment, crisis intervention, counseling, and information and referral, and other miscellaneous duties as needed. Serves as the primary grievance official in the Health Center. This position is a dual role at both of our Brazos Presbyterian Homes locations: The Hallmark and Brazos Towers.

Marginal Functions
- Performs other duties as assigned by authorized personnel or as may be required to meet emergency situations.
Knowledge and Skill
- Bachelor's or Master’s Level Social Worker with at least two years of experience in Long Term Care.
- Must possess general knowledge of Federal and State Regulations, OBRA laws, MDS and the Care Plan process, along with a strong understanding of the needs of geriatric residents.
Qualifications
- Must be able to read, write and follow written and verbal directions.
- Demonstrates skills and ability in working with and understanding the needs of the residents, families, other staff members and personnel from community agencies.
- The ability to present concise, meaningful written reports and articulate the social/psychological needs of the residents.
- The ability to accept and utilize professional supervision, consultation and in-service training.
- The ability to handle confidential data with professional discretion.
- Experience in long term care setting.

- For the safety of associates and residents, the associate is required to follow the Brazos Presbyterian Homes, Inc. "No-Lift Policy". In all resident care situations, staff must utilize mechanical lifting devices (i.e. Hoyer lift, etc.) and avoid manual lifting or handling of our residents. Training will be provided to all associates that will use the mechanical lifting devices at Brazos Presbyterian Homes, Inc. Associate must be able to work in a 24-hour operation.
