Who You Are:
We are seeking a candidate with a strong positive attitude toward customer service to act as a MDS Coordinator (RN). This person will have a genuine interest in working with older adults and be able to engage, motivate, and support both residents and fellow team members. Essential Functions:

1.    Coordinate the IDT for Care Planning, assuring the highest quality of care for residents is developed at and above CMS guidelines.
2.    Timely MDS completion per RAI manual. Giving special attention to completion of forms.
3.    Timely completion of RAI process and RAP summary. Identification of problems, goals, and approaches.
4.    Proper and accurate response to regulation changes and evaluation of the Care Plan system.
5.    Assure timely signatures are obtained.
6.    Effectively communicate with nurses and IDT on the current status of Care Plans.
7.    Responds timely and in writing to deficiencies in MDS audits.
8.    Assist with admission process.
9.    Consistently demonstrate hospitality behavior and the core values of quality, respect, service and integrity.
10.    Maintain work area and keep the department in compliance with OSHA safety standards.
11.    Effectively communicates both verbally and in writing.
12.    Demonstrates the community hospitality promises towards residents, families, associates and vendors.
 

1. Minimum Registered Nursing degree. MDS experience required. Two years of experience working as a registered nurse including some medical/surgical experience and awareness of the needs of geriatric residents.
