The Nurse Care Manager is a licensed Registered Nurse who serves as a clinical guide, educator, and advocates for patients undergoing Hepatitis C treatment. This role combines expert care coordination with targeted education to ensure patients and families are supported throughout the treatment journey—from diagnosis to cure. Acting as a central liaison between patients, providers, and community resources, the Nurse Care Manager plays a critical role in improving access, adherence, and outcomes. The Nurse Care Manager (NCM) is responsible for coordinating the physical and behavioral health needs of individuals with HCV and other health conditions, ensures a client-centered, low barrier approach to HCV treatment, and problem solves any issues participants may have in accessing HCV treatment medications. Requirements
The Nurse Care Manager will:
- Problem solves any issues participants have in accessing HCV treatment medications.
- Coordinate physical and behavioral health needs of individuals with HCV and other health conditions
- Serve as the primary point of contact for patients enrolled in the Rapid Hepatitis C Treatment Program.
- Coordinate all aspects of the treatment plan, including appointments, labs, referrals, and follow-ups.
- Conduct weekly check-ins with patients to monitor progress, address concerns, and reinforce adherence.
- Identify and mitigate barriers to care such as transportation, insurance, or psychosocial challenges.
- Communicate with pharmacies and insurance companies to ensure medication delivery
- Create prior authorizations for medications if needed.
- Collaborate closely with medical and nursing teams to ensure seamless, patient-centered care delivery.
- Work closely with program manager and supervisor to integrate all aspects of hep C, providing patient-centered care.
- Track treatment milestones and confirm sustained virologic response (SVR) post-treatment.
- Currently licensed registered in the state of Delaware
- Bachelor of Science in nursing (BSN) - preferred
- Basic Life Support (BLS) certification - required
- Experience in patient education, program development
- Minimum 2 years of clinical nursing experience, knowledge of Infectious disease and hepatology
- Ability to work in various locations throughout the state of Delaware
- Must have own vehicle for use
- Starting at $73,000
- Commensurate based on experience and education!
- Monday - Friday
- 7 am to 3 pm
