Social Services Coordinator (Part-Time) - Healthcare Community

At Brandermill Woods, we are dedicated to providing compassionate, resident-centered care within our continuing care retirement community. We are currently seeking a Social Services Coordinator to support the emotional, psychosocial, and transitional needs of residents in our healthcare community, including long-term care are skilled nursing.
This is an ideal opportunity for someone who enjoys meaningful, relationship-driven work and wants to play an active role in resident care planning and interdisciplinary collaboration. This position offers flexibility while maintaining a consistent weekday schedule.

This is more than a support role, you will be a key contributor in care planning, resident advocacy, and family support. You will work closely with residents, families, and clinical teams to help ensure each resident's voice is heard and their needs are met.
Schedule
- Part-Time - 25 hours per week
- Monday - Friday (flexible within weekday hours)
(Consistent weekday availability is important for care plan meetings and team collaboration.)
Key Responsibilities
- Support residents and families through admissions, transitions, and ongoing care needs
- Assist with care plan development and participate in interdisciplinary care plan meetings, particularly for long-term care residents
- Conduct social histories and psychosocial assessments
- Provide emotional support and resource coordination for residents and families
- Collaborate with nursing, therapy, and leadership teams to ensure holistic care
- Assist with discharge planning and continuity of care
Qualifications
- Bachelor degree in Social Work, Psychology, Sociology, or related Human Services field preferred
- Experience in long-term care, skilled nursing, or healthcare setting strongly preferred
- Demonstrated initiative and ability to work independently within a team environment
- Strong communication, organization, or interpersonal skills
- Experience with PointClickCare (PCC) or similar EMR system is a plus
