What are the responsibilities and job description for the Social Worker/Case Manager position at Brain Injury Association of Rhode Island?
Overview
The Brain Injury Association of Rhode Island (BIARI) is seeking a compassionate and dedicated Social Worker/ Case Manager (SW/CM) to join our team. Under the supervision of the Senior Resource Facilitator (SRF), this individual will provide resource facilitation, including information, referral, education and support to individuals diagnosed with brain injury and their families/caregivers. This extends to database management, home visits, special projects, and support group facilitation.
The SW/CM will work proactively and collaboratively as part of the office team and, as a representative of BIARI, must maintain a professional manner and assist in fostering a culture of caring and engagement philanthropy with all constituents of the Association.
Nature and Scope of Position
- Work both independently and collaboratively
- Keep informed and updated regarding brain injury, resource access and best practices
- Design and develop spreadsheet/databases to capture client information
- Track the organization’s progress in meeting and supporting core program goals and objectives
- Grow facilitate support groups for brain injury survivors, caregivers and other identified groups
- Under the supervision of the SRF, coordinate and integrate community support services to achieve maximum client-centered outcomes
- An understanding of Grants’ Management and Reporting processes would be helpful
Qualifications
This position requires demonstrated organizational and communication skills both oral and written, ability to communicate with a broad and diverse community with a focus on individuals and families dealing with the impact of brain injury. Bachelor’s degree in social work, counseling, certification in case management or related field, or equivalent experience.
Significant Skill Set Preferences
- Understanding the needs/challenges of individuals dealing with the consequences of brain injury.
- Pro-active approach to outreach and resource development strategies and case management best practices, particularly in relation to brain injury.
- Excellent communication and presentation skills, both verbal and written, to ensure effective interaction with clients and professionals.
- Proficiency in Spanish a plus.
- Strong ability to provide compassionate understanding and support to survivors and their families and/or caregivers.
- Proficiency with Microsoft 365, including, but not limited to, Word, Excel, PowerPoint.
- Competent in preparing reports, spreadsheets and file migration, among other things.
- Client-focused approach, and demonstrated ability to respond in a timely and proactive manner to constituent needs.
- Requires current drivers license.

Pay: $39,000.00 - $42,500.00 per year
