The Referral & Intake Support Specialist serves as the primary point of contact for families during the Early Support for Infants and Toddlers (ESIT) referral and intake process, guiding them through initial enrollment with clarity and compassion. This role works closely with multidisciplinary teams, referral partner, and families to coordinate intake appointments, manage documentation, and support timely access to services.

Essential Duties & Responsibilities
The Referral & Intake Support Specialist serves as the primary point of contact for families during the Early Support for Infants and Toddlers (ESIT) referral and intake process, guiding them through initial enrollment with clarity and compassion. This role works closely with multidisciplinary teams, referral partner, and families to coordinate intake appointments, manage documentation, and support timely access to services.

Essential Duties & Responsibilities
- Serve as a welcoming point of contact for families and referral sources
- Receive referrals and gather required intake information
- Coordinate intake evaluation appointments and interpreter services
- Facilitate family enrollment orientations and explain ESIT services and procedural safeguards
- Prepare and manage intake paperwork and enrollment documentation
- Maintain accurate electronic records in Raintree and the ESIT Data Management System (DMS)
- Connect families who are not eligible for ESIT with appropriate community resources
- Collaborate closely with Family Resource Coordinators and multidisciplinary teams to support smooth transitions into services
