Admissions and Referral Intake Specialist

**Description** The Admissions & Referral Intake Specialist supports admissions to the facility, in-house transfers and discharge processing for the Skilled Nursing, TCU and Assisted Living programs. Works in a team-based setting to work collaboratively with hospitals, skilled nursing facilities, referral agencies, discharge planners, social workers, and in-house staff to achieve budgeted census levels. Assists the Mount Leadership Team in outreach activities to enhance the ministry visibility with the greater community. Providence caregivers are not simply valued – they’re invaluable. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
