The Community Health Worker serves as a bridge between patients, healthcare providers, and community resources to reduce barriers to care. This role supports patient-centered care through health education, care coordination, and identification of social factors affecting health, such as housing, food access, and transportation. The CHW works collaboratively with patients, families, and providers to promote preventive care, manage chronic conditions, and support adherence to treatment plans. Must be CMA certified.

Duties/Responsibilities

Below are the primary duties and essential functions of an employee in this position. While not all tasks may be performed by every employee in this role, all employees are expected to perform related tasks as needed. These essential functions must be performed with or without reasonable accommodation.

  • Prepare accurate and comprehensive patient charts for medical appointments, identifying care gaps, barriers to care, and relevant prior test results.
  • Track and coordinate patient care–related data, including medication assistance and samples, drug utilization evaluations, and other relevant care documentation.
  • Complete prior authorizations for medications and referrals as needed.
  • Track and manage closed-loop referrals, ensuring timely follow-up and accurate documentation.
  • Schedule referral appointments while patients are on-site to support continuity of care.
  • Coordinate return appointments across medical, behavioral health, and dental services.
  • Adhere to standard precautions and OSHA guidelines for the safe handling and disposal of medical waste.
  • Administer social needs and social determinants of health (SDOH) screenings as assigned.
  • Provide patients with referral information and community resource lists during on-site visits.
  • Participate in daily huddles to support care management, communication, and coordination efforts.
  • Receive warm handoffs from providers and deliver individualized patient education and support.
  • Document all patient interactions accurately and in real time within the electronic health record.
  • Track SDOH scores, referral completion, no-shows, and rescheduled appointments, conducting outreach to patients requiring follow-up.
  • Maintain clear, consistent, and HIPAA-compliant documentation and professional boundaries.
  • Assist with Uniform Data System (UDS) measures, including tobacco cessation, weight management, and depression screening follow-up.
  • Support Food Is Medicine initiatives by identifying patient needs, verifying eligibility, and completing required documentation.
  • Attend all required meetings, training, and educational sessions.
  • Complete continuing education requirements to maintain certification and compliance.
  • Maintain working knowledge of clinic workflows and processes to assist with direct patient care coverage as needed.
  • Other duties as assigned.

Physical Requirements

  • Occupational Exposure Risk Classification: The normal work routine involves moderate exposure to blood, body fluids or tissues.
  • The normal work routine has a moderate ergonomic risk for repetitive motion impairment.
  • The position may require a moderate amount of physical exertion (bending, lifting, twisting) in the work area.

Required Qualifications

  • Education: High School diploma or General Equivalency Degree (GED).
  • Experience: Minimum of one (1) year working in a medical office setting.
  • Certificate or Professional License:
    • Certification in Medical Assisting through the American Association of Medical Assistants (AAMA) or Commission on Accreditation of Allied Health Education Programs (CAAHEP) Required.
    • Community Health Worker Certification preferred.
    • CPI (Crisis Prevention Intervention) - certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
    • BLS (Basic Life Support) and First Aid - certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
