- Provide comprehensive primary care services, including preventive care, acute visits, and chronic disease management.
- Perform patient evaluations, physical examinations, and appropriate diagnostic assessments.
- Develop and implement individualized treatment plans based on clinical findings and patient needs.
- Order, review, and interpret diagnostic tests and laboratory results.
- Educate patients on preventive health, disease management, and treatment options.
- Collaborate with physicians, specialists, and other members of the care team to coordinate patient care.
- Maintain timely, accurate, and complete documentation in the electronic health record.
- Follow established clinical guidelines and evidence-based practices to support quality patient outcomes.
- Successful completion of the educational requirements for a Nurse Practitioner or Physician Assistant through an accredited program.
- Current, unrestricted NP or PA license in the state of practice, or eligibility to obtain licensure prior to employment.
- Minimum of one year of clinical experience as an NP or PA in primary care.
- Strong clinical judgment, communication skills, and commitment to patient-centered care.
- Ability to work collaboratively with providers and clinical staff in a team-based primary care setting.
- Experience using electronic health record systems.
