Established in 1970, Boley Centers, Inc., is a private, non-profit organization serving individuals with mental disabilities, individuals and families who are homeless, Veterans and youth in Pinellas County.

JOB SUMMARY

This is a temporary grant funded position lasting at least 3 years.

This position is a staff position for TIEH (Treatment of individuals experiencing homelessness). This is a collaborative program located at the Pinellas County Sheriffs office Emergency Shelter (Safe Harbor). This is funded through Pinellas county health and Human Services and is a collaborative effort by Boley Centers, PCSO, Pinellas county, and Eleos (formerly PEMHS). The position will assist clients in securing appropriate Services including by not limited to vocational support, medical support, finding community supports and services, assisting with medication and psychiatric follow up and providing recovery services. Additionally, certain metrics will be monitored, and assistance will also entail social entitlements such as food stamps, Medicaid, social security, and connecting clients to housing supported programs.

JOB FUNCTIONS

  • Completes client intake, admission, and discharge to the program.
  • Complete the assessment, mental status, medical/dental health, use of drugs or alcohol; education and employment; social development and functioning, activities of daily living, family structure, baker act history, and incarceration history.
  • Conducts other as needed functional assessments to determine strengths, needs and preferences.  
  • Maintains a caseload and provides direct services including biopsychosocial assessment, recovery planning, case management, and housing supports.
  • Works with clients to develop a comprehensive service plan to initiate housing referrals and connections. Implements service plans for each assigned client; updates plan month.
  • Coordinates both in-house and/or community appointments, i.e. medical, dental, therapy.
  • Makes and/or receives calls regarding a broad spectrum of client related issues and involving contact with a wide variety of individuals (parents, friends, etc.), social services (Medicaid, Social Security, etc.) and service providers both internal and external.
  • Develops and monitors safety plans. 
  • Maintains formal and informal working relationships with local service and treatment providers to ensure client access to services and treatment.
  • Completes, in an accurate and timely manner, all documentation required to ensure ongoing compliance with all internal and/or external requirements.
  • Maintains ongoing awareness of each assigned client's needs by the frequent reading of such documentation as activity reports, progress notes, etc. and by frequent personal meetings with each client.
  • Operates own or agency vehicle to transport clients and/or travel between locations.
  • Writes memos, letters, statements, etc. regarding client related issues.
  • Will comply with Compliance Plan and standards of conduct and report any noncompliance to the appropriate official.
  • Will attend all required trainings and complete other duties as assigned.

EDUCATION AND EXPERIENCE

Bachelor's degree with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and has a minimum of one year of full time or equivalent experience working with adults experiencing serious mental illness; or bachelor's degree and three years full time or equivalent experience working with adults experiencing serious mental illness.  Education must be from an accredited school, college, or university.

Minimum of one to three years experience working with serious mental illness required depending upon type of bachelor's degree.

Knowledge of mental illness and symptoms, mental health system and supports, community social services systems.

SPECIAL REQUIREMENTS

Valid Florida driver's license, own transportation vehicle and vehicle insurance required.

Must pass a level II background check and drug screen.
