ABOUT THE ORGANIZATION:
Established in 1970, Boley Centers, Inc., is a private, non-profit organization serving individuals with mental disabilities, individuals and families who are homeless, Veterans and youth in Pinellas County.
Boley Centers, Inc., a nationally recognized and accredited behavioral health care agency, has an opening for a full-time Homeless Outreach Coordinator who will be responsible for engaging individuals experiencing homelessness, building trusting relationships, while assessing needs and working collaboratively to identify programs and services to reduce barriers to stability and promote long-term housing solutions.

DUTIES/RESPONSIBILITIES:
- Engages and conducts outreach with homeless individuals who have been in homeless shelter and/or street homeless for 12 months or more of the most recent past 3 years.
- Develops and maintains relationships with community partners and referral sources.
- Conducts needs assessment and program/service area identification.
- Completes homeless screening and verification forms for potential admission into programS or services with the expectation of face to face; however, when needed, accommodations can be made to collect information via phone and or Zoom.
- Communicates with and coordinates potential referrals with program supervisor and administration.
- Tracks referrals from the community and our access center through the homeless referral system and verification log.
- Enters demographics into Avatar under Homeless Pre-Admit.
- Ensures smooth transitions from shelter/street to program admission.
EDUCATION & EXPERIENCE:
- Bachelor's degree in human services or related field required. Master’s degree preferred. No substitution permitted. Education must be from an accredited school, college, or university.
- At least three years of relevant experience in human services with some experience working with mentally ill adults, dual diagnosis and homeless preferred.
- Knowledge of trauma, severe persistent mental illnesses, counseling techniques, behavior modification, medications, substance abuse, rehabilitation and treatment for the homeless population required.
SPECIAL REQUIREMENTS:
Valid Florida driver's license required.
Must pass a level II background check and drug screen.
