We are seeking a Skilled Nursing Facility (SNF) Physical Therapist to provide patient-centered care focused on restoring mobility, independence, and quality of life for short-term rehab and long-term care patients.
Responsibilities include evaluating and treating patients recovering from illness, injury, or surgery, developing individualized plans of care, delivering 1:1 skilled therapy, collaborating with interdisciplinary teams, documenting progress, and educating patients and families on mobility and safety.
Qualifications include a Doctorate or Master's Degree in Physical Therapy, an active (or pending) state PT license, and preferably SNF experience. Strong clinical reasoning and communication skills are essential.
