Role: RN Patient Care Navigator – Stroke Program
Responsibilities: Provide professional nursing care to support neurologists and multidisciplinary teams in managing stroke patients, focusing on acute stroke alerts, interventions, and timely care. Conduct neurological assessments, follow treatment protocols, assist with patient transport, and serve as a resource for stroke protocols, quality initiatives, and compliance. Educate patients, families, and staff on stroke care. Collaborate with the stroke team to meet center standards and maintain stroke program data.

Qualifications
- Bachelor's Degree in Nursing
- Minimum three years of RN acute care experience with stroke patients
- Current Illinois RN licensure
- CPR (BLS) certification; ACLS certification within 3 months of hire
- NIHSS certification
