Role: Private Duty Licensed Practical Nurse (LPN/LVN) specializing in adult trach/vent care.

Responsibilities: Provide compassionate, high-quality homecare for adult patients with trach and ventilator needs. Ensure clinical outcomes through skilled nursing interventions, documentation via electronic systems, and adherence to safety protocols. Support patient comfort and safety while collaborating with the healthcare team.

Qualifications & Skills

  • Active, unencumbered LPN/LVN license in the practicing state
  • Current CPR certification with hands-on component
  • TB skin test within the last 12 months
  • Reliable transportation
  • Preferred: Six months of hands-on nursing experience

Key Duties: Deliver in-home nursing care, manage trach/vent patients, maintain accurate documentation, and provide patient-centered support. Utilize technology for documentation and communicate effectively with team members.
