- Assess, diagnose, and treat patients
- Prescribe medications and manage pharmaceutical treatments
- Maintain electronic medical records
- Lead and coordinate care teams
- Work with leadership to achieve health quality objectives
- Perform other assigned duties
- MD or DO degree with board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state license and DEA license
- US work authorization
- Fellowship in Geriatrics and experience with older adult populations
- Experience in value-based care models and outpatient primary care
- Bilingual proficiency where applicable
- Collaborative approach to achieve positive health outcomes
