We are seeking a skilled physical therapist to develop personalized, measurable care plans tailored to patient needs, collaborating with physicians, families, and healthcare teams. The role involves performing comprehensive evaluations, administering treatments per the plan of care, and documenting assessments promptly. Responsibilities include

• discharge planning,
• supervising physical therapy assistants and aides, and
• educating patients and families about treatment goals.
  
  

Qualifications include a Bachelor's Degree in Physical Therapy, state licensure, and CPR certification. Experience in physical medicine, rehabilitation, and home health is preferred. The role requires a driver's license and auto insurance.

Travel may be up to 75%. The position emphasizes compassionate, comprehensive care and adherence to professional standards.
