Overview
Oversee the delivery of services specifically aligned to various goals in the Individual Service Plan (ISP) of enrolled adults with different abilities who have NOW/COMP Waiver services. Collaborative and direct service roles. Required to participate in all mandated initial and ongoing trainings aligned to compliance for the position.
Responsibilities
- Monitor delivery of ISP goals that are aligned to day program services
- Develop and support strategies that reflect the goals and objectives of the ISP
- Monitor activity data tracking and goal progress to impact timely report building
- Recommend modifications to strategies when needed, provide guidance on specific materials/resources to advance goal progress and assess individual needs in collaboration with day program team members
- Collaborate on content, timelines and process for effective family communication
- Engage in the agency's team process for data collection, entry, reporting & use
- Prepare compliant and routine progress and annual reports, as per procedures
- Conduct functional assessments such as behavioral, intensity level or health risks, and supervise high intensity services that address health and safety risks
- Participate on intake team for new enrollees when waiver services apply, and as otherwise directed to support individualized services to all enrollees.
Qualifications: Any of the following professions
- Advanced Nurse (registered, licensed in Georgia) or Registered Nurse (BS)
- Behavior Specialist with Master's degree in psychology, school psychology, counseling, vocational rehabilitation or related field with specific requirements
- Board Certified Behavior Analyst (BCBA) who has completed a Master's degree
- Educator with degree in Special Education OR teaching Certificate in Special Education AND 1 year of classroom experience in teaching individuals with intellectual/developmental disabilities
- Human Service Professional with bachelor's degree in social work OR human services field AND specialized training OR 1 year experience serving individuals with intellectual/developmental disabilities
- Master's or Doctoral in one of the behavioral OR social sciences AND specialized training in developmental disabilities
- Physical or Occupational Therapist who has training in developmental disabilities
- Physician or Physician's Assistant with experience with developmental disabilities
- Psychologist with related experience
- Speech Pathologist or Audiologist with experience with developmental disabilities
- Therapeutic Recreation Specialist with specialized training or experience
Job Type: Part-time
Pay: From $22.00 per hour
Expected hours: 3 – 5 per week
