Role Description
This is a full-time hybrid role for a Case Manager. The Case Manager will be responsible for assessing clients' needs, developing individualized care plans, coordinating with healthcare providers and services, monitoring client progress, and adjusting care plans as necessary. This role requires consistent communication with clients, families, and multidisciplinary teams to ensure optimal care and outcomes.
