Role Description. This is a full-time, on-site Patient Care Coordinator role based in San Francisco, CA. The Patient Care Coordinator will be responsible for answering incoming calls, responding to inquiries, and demonstrating professional phone etiquette while providing clear information about services and appointments. This role will coordinate patient care by scheduling and confirming visits, gathering necessary medical and dental history, and collaborating with clinical and administrative team members to support house call operations. Daily tasks include managing appointments, verifying patient information, assisting with intake and follow-up communications, and documenting interactions accurately in practice systems. The Patient Care Coordinator will also support patients, families, and caregivers by explaining procedures and processes in an understandable way and ensuring a positive experience.
Qualifications
- Strong appointment scheduling and calendar management skills, including the ability to coordinate complex visits and follow-ups.
- Professional phone etiquette and customer service abilities, with clear, empathetic, and respectful communication.
- Experience with care coordination and patient care, ideally in a dental, medical, or home health setting.
- Working knowledge of medical terminology and comfort learning dental-specific terms and procedures.
- High level of organization, attention to detail, and accuracy in documentation and data entry.
- Ability to collaborate with multidisciplinary teams, including clinicians, caregivers, and administrative staff.
- Comfort working with older adults and individuals with special needs, demonstrating patience and cultural sensitivity.
- Proficiency with basic office software and practice management or electronic health record systems (or willingness to learn).
