Position Title: MDS Coordinator
Salary: $100,000 to $105,000
Schedule: Full Time, 9:00am - 5:00pm, Monday- Friday

RN currently licensed by State of Maryland. Current CPR certification. In- depth experience and knowledge working with MDS computer software. Experience with the MDS transmittal process and understanding of regulations impacting the MDS. Supervisory experience preferred.
All new hires must be able to pass our preemployment screening process, which includes a fitness for duty physical, criminal background check, drug test and employment references as required by a licensed residential care facility. Some of the roles in our community also require that we ask about your vaccination status.

Primary Responsibilities
Essential Job Duties:
- Coordinates, conducts, and completes the MDS on the computerized system,
residents.
- Assures accuracy and completeness of the medical record.
- Assures and monitors that MDS’s are completed within state and federal
- Ensures MDS electronic submission occurs per State regulations.
- Coordinates and participates in family/resident care plan conferences and
- Assures effective communication with other shifts and departments as it relates
- Communicates RUGS levels with accounting department to ensure accurate
- Responsible for preparing and submitting OCAP and Benchmark reports.
- Monitor Relias training to ensure compliance for all Chestnut Green staff.
- Conduct needed education sessions and ensure that all applicable staff
Other Duties
- Serves as a staff nurse when staffing levels necessitate in order to meet the
- Responds to emergency calls as necessary.
- Assures timeliness of physician’s visits.
- Notifies physicians and family of changes in resident’s condition.
- Encourages rehabilitative techniques to restore residents’ independence.
- Assists DON in developing and implementing medical records policies and
- Attends in-service training and education sessions, as assigned.
- Performs specific work duties and responsibilities assigned by supervisor.
- Participate in morning meeting each day and ensure that the care plan is kept up
- Participate in the monthly Quality Assurance meeting by preparing and
