Summary Of The Role
The on-call Social Worker provides biopsychosocial/spiritual assessments, diagnoses, treatment, advocacy, and develops person-centered care plans in collaboration with patients and multidisciplinary teams.
For MSW candidates: requires a Master’s Degree and an Oregon Clinical Social Work Associate License within 60 days. Preferred experience includes two years in healthcare, hospice, or bereavement settings.
For LCSW candidates: requires a Master’s Degree from an accredited school, licensure in Oregon or Washington, and two years of relevant experience, including care management in acute care preferred.
Key duties: perform assessments, develop care plans, collaborate across departments, respect patient self-determination, reduce disparities, and adhere to NASW standards and ethics.
