Physical Therapist (PT) - Full-time

Role: We are seeking a Physical Therapist (PT) to provide exceptional home-based patient care in Greeley, CO and surrounding areas. The position offers full-time and part-time opportunities, with visit rates ranging from $75-$150 plus mileage and travel time reimbursement.

Responsibilities

• Conduct initial and ongoing comprehensive assessments
• Provide therapeutic interventions and treatments
• Communicate effectively with interdisciplinary team members to coordinate care and discharge planning
• Participate in Quality Assessment and Performance Improvement (QAPI) programs
• Report on patient conditions to the Rehab Director and staff
  
  

Qualifications And Skills

• Current PT license in Colorado
• Basic Life Support (BLS) CPR certification or willingness to obtain
• At least one year of PT experience
• Home health experience preferred but not required
• Ability to pass background checks and drug testing
