Role Overview
Provide comprehensive in-home health assessments to evaluate members' health, medical history, medication adherence, social environment, and risks during visits lasting up to one hour. Collaborate with healthcare professionals, administer tests, and document findings to improve care quality and identify care gaps.

Provide comprehensive in-home health assessments to evaluate members' health, medical history, medication adherence, social environment, and risks during visits lasting up to one hour. Collaborate with healthcare professionals, administer tests, and document findings to improve care quality and identify care gaps.
Responsibilities Include
- Conduct adult/geriatric assessments, including medical history and treatment plans
- Administer and order point-of-care and lab tests
- Coordinate with physicians, case managers, social workers, and caregivers
- Provide education and follow-up in partnership with primary care providers
- Perform services at members' homes or facilities
- Master's Degree or equivalent experience with NP licensure
- Current state licensure with prescriptive authority
- Board certification in relevant specialty (e.g., Adult Gerontology, Family)
- 1 year NP experience, BLS/ACLS or CPR certified
- Ability to travel extensively, valid driver’s license, and insurance coverage
- Strong computer skills and excellent communication abilities
