Provide comprehensive health assessments in members' homes or facilities, reviewing medical history, medications, social factors, and risks to enhance care quality and address care gaps.
Responsibilities
- Conduct adult/geriatric assessments, including documentation, health education, referrals, and follow-up.
- Administer and order point-of-care and lab tests.
- Collaborate with physicians, case managers, social workers, and caregivers.
- Coordinate with primary care providers for patient education and follow-up.
- Work at members’ residences, including homes and Skilled Nursing Facilities.
- Master’s Degree or equivalent, with NP licensure and prescriptive authority.
- Board certification in relevant specialties (e.g., Adult, Family, Gerontology).
- Current BLS, ACLS, or CPR certification.
- At least 1 year of NP experience.
- Ability to travel extensively, including overnight and weekends.
- Valid driver’s license, auto insurance, strong computer skills, and excellent communication.
