Key Responsibilities Include
- Delivering quality patient care under RN supervision, documenting services, and communicating with physicians and caregivers.
- Recognizing patient condition changes, implementing interventions to mitigate risks, and educating patients and families based on individual needs.
- Performing various home care skills such as wound care, infusions, catheter care, PICC line dressing changes, and phone triage.
- Active LPN license in the applicable state.
- Valid driver’s license, auto insurance, and CPR certification.
- Preferred: Experience with Home Care Home Base platform.
