Responsibilities
- Conduct comprehensive evaluations of patients’ physical conditions, including mobility, strength, flexibility, and pain levels.
- Develop individualized care plans based on patient assessments, medical history, and specific needs.
- Implement therapeutic interventions such as manual therapy, trigger point therapy, exercise programs, and modalities to promote healing and functional recovery.
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient care transitions.
- Maintain accurate and detailed clinical documentation in compliance with healthcare standards and electronic health records (EHR) systems.
- Educate patients and their families on injury prevention, physiotherapy exercises, and home care strategies to support ongoing recovery.
Experience
- Proven experience practicing in a physical therapy role within outpatient clinics or hospital environments.
- Demonstrated knowledge of physiology, anatomy, medical terminology, and clinical documentation standards..
- Experience managing care plans for acute care patients requiring discharge planning or post-surgical rehabilitation.
- Proficiency in electronic health records (EHR) or electronic medical records (EMR) systems for efficient documentation and patient tracking.
- Ability to work effectively with interdisciplinary teams to deliver high-quality patient care across various clinical environments.
