What You Can Expect
As the LPN Clinical Supervisor, you will serve as a key leader for our nursing and direct care teams by:
- Provide daily leadership, guidance, and support to nursing and direct care staff.
- Foster a positive, accountable, and high-performing team culture.
- Assist with hiring, onboarding, staff evaluations, coaching, and corrective action.
- Lead staff education, training initiatives, and competency development.
- Conduct resident rounds and oversee quality clinical care delivery.
- Coordinate admissions, transfers, assessments, and care planning activities.
- Partner with physicians, hospice providers, home health agencies, and families to support resident needs.
- Ensure timely and accurate clinical documentation and physician order management.
- Oversee medication management processes and regulatory compliance.
- Facilitate care plan meetings and maintain person-centered care plans.
- Monitor quality assurance initiatives, clinical audits, and performance improvement activities.
- Respond to resident, family, and team concerns with professionalism and urgency.
What We Need In A Candidate:
- Current South Carolina LPN license in good standing.
- Minimum one year of long-term care, assisted living, or senior care experience.
- Demonstrated leadership, coaching, and team development skills.
- Strong clinical judgment and organizational abilities.
- Experience working with electronic medical records (EMR).
- Proficiency with Microsoft Office (Word, Excel, Outlook).
- Excellent communication and relationship-building skills.
