MDS / Care Plan Coordinator
Birkwood Village – Fort Madison, IA
Pay: Starting at $45/hour | Department: Nursing | Reports to: Director of Nursing
Schedule: Full-time, on-call rotation every 4th weekend
About the Role
Birkwood Village is hiring an experienced MDS / Care Plan Coordinator to lead resident assessments (MDS) in accordance with current rules, regulations, and RAI guidelines — a key clinical leadership role shaping quality of care across our community.
Responsibilities
- Coordinate MDS assessments, ARDs, and timely submission; stay current on RAI Manual changes
- Review census changes, progress notes, and Medicare A documentation for significant changes in condition and skilled services
- Develop and coordinate individualized care plans, ensuring timely completion and revision with every assessment or status change
- Attend weekly Medicare meetings and monthly/quarterly QAPI meetings
- Promote patient- and family-centered care, including scheduling Care Plan Conferences
- Participate in on-call rotation, every 4th weekend
Qualifications
- Current Iowa RN license in good standing
- MDS/RAI, PDPM, and care plan coordination experience preferred
- Familiarity with PointClickCare (PCC) or similar software a plus
- Strong organizational skills and attention to regulatory deadlines

Pay: $45.00 - $48.00 per hour
