Position Details
- Full-time, in-person position
- Assigned to a high school transition program
- Work with students preparing for post-secondary education, employment, and independent living
- Manage a caseload of students requiring social-emotional and behavioral support
- Collaborate closely with teachers, administrators, special education staff, and community agencies
- Strong administrative and multidisciplinary team support
- Provide individual and group counseling services to students.
- Support students' social-emotional, behavioral, and transition-related needs.
- Participate in IEP meetings and contribute to transition planning and goal development.
- Conduct assessments and collect data related to student social-emotional functioning.
- Collaborate with families, school personnel, and community resources to support student success.
- Assist with crisis intervention and behavioral support planning as needed.
- Maintain accurate documentation and ensure compliance with state and federal regulations.
- Help connect students and families with community-based services and post-secondary resources.
- Valid Illinois Professional Educator License (PEL) with School Social Work endorsement.
- Experience working in school-based settings preferred.
- Knowledge of special education laws, procedures, and transition planning.
- Strong communication, collaboration, and case management skills.
- Ability to work effectively as part of a multidisciplinary team.
