Job Overview
We are seeking a Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals by providing guidance, resources, and advocacy. This position requires a strong commitment to improving the lives of clients through effective case management practices. This includes assisting clients gain access to needed medical, social and educational resources. This includes collaborating with therapists and counselors on identified goals on Individual Treatment Plans, networking with and collaborating with rural community partners, and linkage to appropriate services.
Duties
- Collaborate with therapists and counselors to contribute to treatment plan updates.
- Responds to clients in need of crisis intervention and coordinates/ refers clients in need of immediate support to appropriate services.
- Coordinates and collaborates with community organizations/ agencies to provide comprehensive coordination of care.
- Conducts community outreach to advocate on behalf of client and program needs.
- Mentor clients through challenges, employing motivational interviewing techniques to encourage positive change.
- Completes documentation of client progress and interactions in compliance with organizational standards.
- Maintains confidentiality of records in accordance with HIPAA and other guidelines.
- Transports clients to accomplish their stated goals- making planned trips to Social Security Office, Driver License Office, Gulf Bend Center, etc.
- Performs other duties as assigned by Outpatient Program Director, Chief Operating Officer, or CEO.
Skills/ Education
- Bachelor's degree in social services or related field OR at least two years' experience providing direct services to individuals in treatment or receiving recovery support services.
- Related field work is preferred.
- Ability to balance travel to rural sites and manage caseload.
- Excellent communication skills with the ability to build rapport with diverse populations.
- Strong organizational skills to manage multiple cases effectively while maintaining attention to detail.
Job Type: Full-time
Pay: From $18.00 per hour
