Position Summary:

The primary focus of the Services and Care Coordinator is to provide support towards permanent and affordable housing resources and connections to appropriate services to Big Valley tribal members who are experiencing homelessness and/or at-risk of homelessness, including individuals who may have complex health or behavioral health conditions. This position will assist and support homeless Big Valley tribal members enrolled in the Big Valley Rancheria (BVR) Housing Department’s housing programs. Overall responsibility involves working directly
with housing program participants, to include developing and coordinating treatment services, assist participants in identifying and securing housing stability, provide housing retention support, and other housing supportive services.

Essential Duties and Responsibilities:

1. In-take of applications and ensures all documentation is accurate and complete.

2. Conduct comprehensive assessments and utilize intake forms to determine appropriate housing options, case management and other supportive service needs.

3. Track assessment and data collection into a management information system for reporting.

4. Develops individualized case plans with measurable goals, objectives, and outcomes.

5. Conduct on-going needs/financial assessments to identify participant’s continued housing plan, supportive service needs and barriers to securing and maintaining permanent housing.

6. Work with program participants to develop a housing stability plan and budget.

7. Works with program participants and the Benefits & Wraparound Specialist to develop a housing service plan and housing location plan.

8. Networks and coordinates with representatives from other agencies and community-based organizations to support tribal members in attaining needed services, such as housing, medical treatment, mental health care, financial assistance, legal advocacy, etc.

9. Educates participants on tenant rights, responsibilities, and how to be a good tenant.

10. Provides tenant support when housing-related issues arise.

11. Identifies long-term affordable housing resources and assists participants to pursue options with referrals and support with filling out applications.

12. Assists participants with transitioning into permanent housing and provides supportive services to improve participants overall housing stability.

13. Performs home visits, eviction prevention, budgeting, life skills coaching, and other housing retention services to support participants with sustaining permanent housing.

14. Prepares, maintains, and updates participant’s case files related to the case for review and recordkeeping and/or reporting purposes. Ensures timely completion of case notes.

15. Compile data, prepare designated statistical reports as required; Prepare and submit monthly updates to the Housing Director.

16. Always maintain program and client confidentiality and integrity.

17. Maintain working relationship with the Fiscal Department and other departments.

18. Any other duties as assigned by the Housing Director.

Qualifications:

Qualifications, Education and Experience:

1. Possess a High School diploma or GED certificate.

2. Possess 2-4 years of experience in human services, social work, or related field.

3. Ability to gather complete information from program participants in both telephone and in-person interviews and have familiarity with principles of harm reduction.

4. Ability to plan, evaluate and prioritize work assignments to ensure accurate and timely implementation and completion as well as work independently and support team efforts.

5. Ability to handle multiple tasks with clear sense of ownership and priority.

6. Must have excellent written and verbal communication skills. Create and track written correspondence with participants and be able to report on caseload progress with experience in case management preferred.

Other Requirements:

1. Must be eligible for coverage under the Tribe’s vehicle insurance policy.

2. Must be proficient with Microsoft Office Suite.

3. Must possess a valid California driver’s license. Must be willing to travel locally,

regionally, statewide, and/or out-of-state, as necessary, during or after regular working hours for trainings, workshops and/or conferences.

4. Work involves periods of sitting, walking, bending, crouching, stooping, stretching, reaching, or similar activities, and lifting of moderately heavy items, up to 25 Ibs.

5. Must be able to satisfactorily complete a background check, drug test, and a pre-employment screening.

Indian Preference:

Preference in filling vacancies shall be given to a qualified Big Valley Tribal Member and then to enrolled American

Indians in accordance with the Indian Preference Act (Title 25 U.S. Code, § 472 and 473). Applicants claiming Indian

preference must submit verification by Tribe of affiliation or other acceptable documentation of Tribal Enrollment.
