- Provide comprehensive Licensed Practical Nurse (LPN) care for residents
- Administer medications and treatments as prescribed by the attending physician and in accordance with facility protocols
- Monitor residents’ health status and promptly report changes in condition to the supervising RN, physician, and appropriate family members or responsible parties
- Provide skilled nursing services within scope of practice, including but not limited to tube feedings, suctioning, catheter care, wound care, dressing changes, and other ordered treatments.
- Accurately complete all required documentation, charting, and reports in accordance with regulatory requirements and facility policies.
- Support and implement individualized resident care plans while ensuring resident dignity, preferences, and rights are respected.
- Observe residents closely and report significant changes in physical, mental, or emotional condition to the interdisciplinary care team
- Collaborate with the interdisciplinary team to ensure coordinated, high-quality resident care
- Schedule: Full-time
- Location: Sheridan, Wyoming
- Active Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) license issued by Wyoming required
- BLS certification from the AHA required
