Join our interdisciplinary hospice team and help patients and families navigate one of life's most meaningful journeys. As a Hospice Social Worker, you'll provide counseling, advocacy, education, and care coordination that truly changes lives.

What You'll Do

As a member of our interdisciplinary team, you will:

• Complete comprehensive psychosocial assessments
• Provide emotional support, counseling, and crisis intervention for patients and families
• Assist with advance care planning and community resource referrals
• Help families navigate Medicaid applications, financial resources, and long-term care planning
• Collaborate closely with physicians, nurses, chaplains, volunteers, and other members of the hospice team
• Participate in interdisciplinary care conferences
• Advocate for patients and families while honoring their goals and wishes
• Provide services wherever patients call home—including private residences, assisted living facilities, nursing homes, hospitals, and inpatient hospice care

SCHEDULE: 

Monday through Friday 8 AM to 5 PM 
On Call Rotation once every 7 weeks and holidays

What We're Looking For

Required Qualifications

• Master's Degree in Social Work (MSW 
• Valid Florida driver's license
• Reliable transportation and current auto insurance

Preferred Qualifications

• Two years of healthcare, hospice, home health, oncology, palliative care, or related experience
• Florida Licensed Clinical Social Worker (LCSW) or license-eligible
• Knowledge of grief and bereavement counseling
• Experience working with interdisciplinary healthcare teams
• Strong communication, organizational, and counseling skills
