What You'll Do
As a Hospice Social Worker, you will:
- Complete comprehensive psychosocial assessments for hospice patients and families
- Provide emotional support, counseling, and crisis intervention
- Assist patients and caregivers with advance care planning, community resources, financial assistance, and long-term care planning
- Advocate for patient wishes and help families navigate difficult decisions
- Collaborate with physicians, nurses, chaplains, hospice aides, volunteers, and other members of the interdisciplinary care team
- Participate in interdisciplinary team meetings to develop individualized plans of care
- Educate patients and caregivers regarding available resources and benefits
- Provide care in homes, assisted living facilities, nursing homes, hospitals, and inpatient hospice settings
- Participate in an on-call rotation as assigned
SCHEDULE:
Monday through Friday 8 AM to 5 PM
On Call Rotation with holiday rotation
Qualifications
Required
- Master's Degree in Social Work (MSW)
- Valid Florida driver's license
- Reliable transportation and current automobile insurance
- Ability to travel throughout Jackson, Holmes, Washington, Calhoun, Gulf, and Bay Counties
Preferred
- Florida Licensed Clinical Social Worker (LCSW) or license-eligible
- Two years of hospice, palliative care, home health, oncology, or healthcare experience
- Experience with grief counseling and end-of-life care
- Knowledge of community resources and government assistance programs
- Strong communication, counseling, and organizational skills
