At Big Bend Hospice, we are dedicated to providing exceptional end-of-life care, ensuring the highest quality of life for our patients and their families. Guided by our core values of integrity, respect, accountability, and excellence, we are seeking a compassionate and skilled RN Case Manager to join our interdisciplinary team for our Coastal (Wakulla and Franklin County) service area. In the role of RN Case Manager, you will provide hospice homecare services and support to our patients, their families and caregivers as they navigate the hospice journey.
As an RN Case Manager, you will:
- Develop, coordinate, and implement individualized care plans for hospice patients.
- Provide comprehensive symptom management and patient/family education.
- Supervise Hospice Aides and Licensed Practical Nurses, ensuring high-quality care.
- Maintain a caseload of up to 18 patients, overseeing all aspects of their care.
- Document accurately and efficiently in our electronic medical record (EMR) system.
- Collaborate with physicians and interdisciplinary team members to set realistic patient goals.
- Participate in quality assurance, staff orientation, and community education initiatives.
- Uphold Big Bend Hospice’s mission, values, and policies while delivering compassionate care.

Education & Experience:
- Bachelor's degree in nursing.
- Valid Florida Registered Nursing License (required).
- Minimum of 2 years of inpatient nursing, hospice, home health, or long-term care experience.

Skills & Abilities:
- Excellent communication, interpersonal, and organizational skills.
- Strong analytical and problem-solving abilities.
- Ability to effectively and efficiently utilize and navigate technology for daily tasks, including patient charting and record keeping. Proficiency with Microsoft Office Suite and electronic medical records.
- Ability to manage physical and emotional demands of caring for terminally ill patients.

Requirements:
- Valid Florida driver’s license, auto insurance, and reliable transportation.
- Physical ability to stand/walk 66-100% of the shift, lift up to 50 lbs., and perform patient care tasks.
