As a Hospice Aide (CNA) – Home Care, you’ll travel throughout the community providing compassionate, hands-on personal care to hospice patients in their own living environments. Working under the direction and supervision of a licensed nurse, you’ll help patients remain comfortable, safe, respected, and supported throughout their end-of-life journey.

This is more than assisting with daily care. You’ll build relationships with patients and families, notice meaningful changes, provide reassurance, and help preserve dignity during one of life’s most vulnerable seasons.

🌿 What You’ll Do

💙 Personal Care & Comfort

• 🛁 Assist patients with bathing, showers, bed baths, and personal hygiene.
• 👕 Assist with dressing and undressing.
• 🪥 Provide oral and denture care.
• 💇 Assist with grooming needs, including hair care and shaving.
• 🧴 Provide skin care and apply approved non-medicated topical products as directed by the licensed nurse.
• 🛏️ Make beds and help maintain a clean, comfortable patient environment.
• 🤲 Provide appropriate comfort measures to promote relaxation, dignity, and patient well-being.
• 🍽️ Assist patients with meals and feeding when needed.
• 🔄 Safely reposition, move, and transfer patients using proper body mechanics and assistance when necessary.
• 🧦 Assist with established supportive devices as directed and within the CNA scope of practice.

🚻 Toileting & Personal Needs

• 🚽 Assist patients with toileting, bedside commodes, bedpans, and urinals.
• 🧼 Provide appropriate perineal and incontinence care.
• 💧 Assist with urinary drainage bags and external catheter care within the CNA scope of practice.
• 📋 Observe and report bowel and urinary changes as appropriate.
• 🩹 Provide established ostomy care as directed and within scope of practice.
• 💙 Provide all personal care with sensitivity, privacy, and respect for the patient’s dignity.

Every patient and every home environment is different, so adaptability, professionalism, and respect are essential.

During your visits, you’ll:

• 👋 Build positive, respectful relationships with patients, caregivers, and families.
• 🩺 Follow the individualized Hospice Plan of Care established for each patient.
• 👀 Observe patients for changes in condition, comfort, behavior, or symptoms.
• 📣 Promptly communicate changes or concerns to the supervising licensed nurse.
• 💙 Pay close attention to patient comfort, pain, positioning, medications, and personal needs.
• 🛏️ Help ensure necessary supplies and durable medical equipment are available and functioning appropriately.
• 💨 Assist with routine oxygen-equipment tasks assigned within your role.
• 🧼 Reinforce patient and caregiver education related to hygiene and personal care.
• 🔄 Reinforce safe repositioning and comfort measures.
• 🚻 Support patients and caregivers with appropriate bowel and bladder care education.
• 👂 Listen compassionately and provide a reassuring presence for patients and families.

🩺 Observation & Communication

Hospice Aides spend valuable one-on-one time with patients, which means your observations matter.

• 👀 Observe and report changes in patient condition, unusual signs or symptoms, and care provided.
• 🩺 Obtain and record assigned vital signs.
• 🚨 Promptly report vital signs or observations outside established parameters to the RN.
• 📝 Complete required documentation accurately and on time.
• 📣 Report incidents, accidents, injuries, or safety concerns promptly.
• 💡 Contact the supervising nurse whenever a patient need or task falls outside your role or requires additional clinical guidance.
• 🤝 Communicate respectfully and professionally with patients, caregivers, nurses, and other members of the hospice team.

🛡️ Patient Safety & Infection Prevention

• 🧼 Follow appropriate hand hygiene and infection-prevention practices.
• 🧤 Use standard precautions while providing patient care.
• 🏋️ Use proper body mechanics when repositioning, transferring, or assisting patients.
• 🏠 Help maintain a clean and safe patient-care environment.
• 🚨 Follow established procedures during urgent or emergency situations.
• 🛡️ Advocate for patient safety, comfort, dignity, and individual preferences.
• 📦 Communicate patient supply or equipment needs to the appropriate team member.

📋 Requirements

✅ Required Qualifications

• 🎓 Successful completion of an approved Certified Nursing Assistant (CNA) program.
• 🩺 Current Certified Nursing Assistant certification in the State of Florida.
• ❤️ Current CPR certification.
• 🚗 Valid driver’s license and clean driving record.
• 🛡️ Current automobile insurance.
• 🚙 Reliable transportation in good working condition for travel to patient locations.
• 💻 Ability to use electronic documentation systems and basic computer technology.
• 🏠 Ability to safely provide care in a variety of home and community environments.
• ❤️ Ability to manage the physical and emotional demands of caring for terminally ill patients and their families.

🌟 Preferred Qualifications

• 🩺 One year of recent clinical experience.
• 💙 Previous hospice and/or home health experience.
• 🕊️ Hospice and Palliative Care Certification.
• 🏠 Previous experience providing direct patient care in home or community settings.

🏡 What to Expect in Home Care

Providing hospice care in the community is different from working in a hospital or facility.

You’ll travel to patients in a variety of living environments. Conditions may vary from one location to another, and the position may involve exposure to:

• 🐕 Household pets and animals.
• 🌡️ Varying indoor and outdoor temperatures.
• 🦠 Communicable illnesses and other conditions associated with direct patient care.
• 🔊 Noise, conversations, activity, and other environmental distractions.
• 🚗 Regular travel between patient locations.
• ❤️ Emotionally challenging situations involving serious illness, death, grief, and loss.

Throughout every environment, we expect our team members to maintain professionalism, compassion, patient safety, and respect.

💪 Physical Requirements

This is an active, hands-on patient-care position.

• 🚶 Ability to stand and walk for significant portions of the workday.
• 🏋️ Ability to lift up to 50 pounds as needed.
• 📦 Ability to carry up to 25 pounds as needed.
• 🛏️ Ability to push or pull patient-care equipment on wheels with assistance as needed.
• 🪜 Ability to navigate stairs when necessary.
• ↕️ Ability to bend, squat, kneel, reach, and perform movements necessary for direct patient care.
• 🤲 Ability to safely assist with patient positioning, mobility, and transfers.
• 👀 Adequate vision and hearing to safely provide patient care, communicate effectively, utilize equipment and mobile devices, and complete required documentation.
