Key Responsibilities
Population Health Registry & Worklist Development
- Design, build, and maintain patient registries within eClinicalWorks to support uninsured and at-risk populations, post-emergency department and hospital discharge follow-up, continuity of care initiatives, and high-risk patient management.
- Develop role-based daily worklists for Community Health Workers, enrollment specialists, front desk teams, and care coordinators.
- Configure real-time registry updates that generate actionable patient lists without requiring manual intervention.
- Continuously refine and expand registry functionality in alignment with project milestones and organizational priorities.
Clinical Workflow Automation
- Configure coverage status alerts and patient flags within eCW workflows to identify at-risk patients at the point of care.
- Standardize patient check-in workflows across all Company locations.
- Design and implement enhanced digital intake capabilities and front desk automation processes.
Outreach Workflow Configuration
- Build outreach tracking tools and documentation templates that support Community Health Worker and enrollment specialist activities.
- Configure automated patient communications, including insurance renewal reminders and appointment notifications.
- Implement emergency department and hospital discharge workflows, including 48-hour outreach tracking and 7-day follow-up monitoring.
Reporting & Performance Monitoring
- Develop dashboards and reports that monitor key performance indicators, including:
- Registry population size
- Worklist completion rates
- Outreach activity volume
- Appointment conversion rates
- Ensure all configurations support organizational reporting requirements and contractual performance measures.
- Collaborate with Data Analytics teams to align eCW outputs with enterprise reporting initiatives.
Project Leadership & Cross-Functional Collaboration
- Participate in project meetings and provide regular build updates to organizational leadership, including the CMIO.
- Document all configurations, workflow designs, and technical decisions to support training, knowledge transfer, and long-term sustainability.
- Partner closely with clinical, operational, nursing, and community health leaders to translate business requirements into effective eCW solutions.
- Identify system limitations and proactively recommend alternative approaches and process improvements.
Required Qualifications
- Minimum of 3 years of hands-on eClinicalWorks configuration, build, and optimization experience within a healthcare environment.
- Demonstrated expertise building patient registries, worklists, templates, alerts, and automated workflows within eCW.
- Experience supporting clinical informatics, population health, or healthcare IT implementation initiatives.
- Strong ability to translate clinical and operational workflow requirements into technical system configurations.
- Proven organizational and project management skills with the ability to manage multiple concurrent workstreams.
- Excellent communication, collaboration, and stakeholder management skills.
- Comfortable working directly with clinical, operational, and executive leadership teams.
