Role Description
Service Coordinator (NHTD/TBI Waiver Program)
We are seeking a dedicated Service Coordinator to support applicants and participants in the NHTD/TBI Waiver Program. This role involves providing case management, advocacy, and coordination of services to ensure participants receive the care and support they need.
Salary:
The hiring range for this position is $60,000 - 75,000.00 annual bonuses.
Various factors will determine final compensation, such as a candidate’s years of relevant work experience, skills, certifications, and location.

8 hour shift - no weekends / Full Time
Responsibilities:
- Develop and implement Initial Service Plans
- Coordinate services between providers and participants
- Conduct monthly/quarterly in-home visits within the 5 boroughs
- Facilitate, orchestrate, and document team meetings
- Provide ongoing participant support and advocacy
- Prepare and submit Renewal Service Plans
Requirements:
- Master's degree and 1 year of case management experience, OR Bachelor’s degree with 3 years of case management experience, OR Associate’s degree with 5 years of case management experience
- Ability to travel throughout the five boroughs (public transit or personal vehicle)
- Bilingual English/Spanish or Russian strongly preferred
- Strong organizational and communication skills
