Health and Wellness Coordinator
Job Opportunity - Part-Time / Temporary
The Diabetes Care Coordinator supports individuals at risk for or living with diabetes who face barriers to accessing healthcare and social services, including geographic isolation, low income, chronic illness, or complex social needs. The Diabetes Care Coordinator provides individualized care coordination, diabetes self-management education, and resource navigation to improve health outcomes and quality of life, improve diabetes self-management skills, and reduce barriers to accessing preventive and ongoing care. The position supports individuals withprediabetes, type 1 diabetes, type 2 diabetes, and other diabetes-related conditions by promoting healthy lifestyle changes, monitoring care engagement, and encouraging long-term disease management.
Key Responsibilities Care Coordination
- Conduct comprehensive intake and ongoing assessments addressing health status, nutrition, behavioral health, and social determinants of health affecting diabetes management.
- Assist clients in understanding the differences between prediabetes, type 1 diabetes, type 2 diabetes, and gestational diabetes.
- Connect clients and assist with navigating community resources including healthcare providers, nutrition services, behavioral health services, housing programs, and social support services.
- Collect relevant diabetes management information such as glucose meter logs, continuous glucose monitoring (CGM) reports, or other health information when available.
- Collaborate and maintain communication with partners, providers and care team members regarding client progress and needs.
- Provide culturally responsive diabetes education and self-management support tailored to individual needs.
- Support clients and educate in developing healthy lifestyle habits related to nutrition, physical activity, medication adherence, blood glucose monitoring and risk for developing diabetes.
- Implement outreach and engagement strategies to recruit and retain clients in diabetes education and wellness programs.
- Support group education sessions, workshops, or wellness classes when applicable.
- Conduct visits in office, community partner sites, or clients’ homes or living environments as appropriate.
- Participate in required trainings, staff meetings, and professional development opportunities.
Skills & Experience Education
- High school diploma or GED required
- Associate’s degree in public health, social services, community health, health education, or related field preferred
- Minimum 3 years of experience in chronic disease management, diabetes education support, or community health programs
- Minimum 3 years of experience in community care coordination, case management, or health education
- Experience working with underserved populations
- Experience using motivational interviewing
Knowledge & Skills
- Understanding of diabetes prevention and management
- Knowledge of risk factors and lifestyle management strategies related to diabetes
- Knowledge of community health resources and social service systems
- Ability to build trusting relationships with clients and community partners
- Excellent interpersonal and communication skills
- Cultural and linguistic understanding and experience working with diverse communities
- Strong organizational and time-management skills
- Ability to manage multiple cases and priorities in a fast-paced environment
- Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint) and data reporting systems
Language
- English required
- Fluency in a second language preferred
Additional Requirements
- Ability to travel between program sites, community events/activities, and client homes
- Valid driver’s license and reliable transportation preferred
PHYSICAL REQUIREMENTS
- Ability to carry light loads (up to 25 lbs.)
- Prolonged standing and /or sitting required.
- Ability to interface with electronic office equipment for extended periods of time.
- Usual office environment with frequent sitting, walking, and standing, and occasional climbing, stooping, kneeling, crouching, crawling, and balancing.
- Frequent use of eye, hand, and finger coordination enabling the use of office machinery.
- Oral and auditory capacity enabling interpersonal communication as well as communication through automated devices such as the telephone.
