Job Title: Wound Care Specialist (Mobile Healthcare Provider)
Location: Multiple Regions Across South Carolina

Position Summary:
We are seeking dedicated and compassionate Wound Care Specialist to join our growing mobile healthcare team serving communities throughout South Carolina. As a provider, you will deliver high-quality, advanced wound care individuals in residential homes, assisted living facilities, group homes, and other non-traditional settings.
This is a mobile role, and candidates must be comfortable traveling within a 30–45-mile radius of their home location.
Key Responsibilities:
- Conduct thorough assessments and clinical evaluations for patients with acute and chronic wounds.
- Develop and implement individualized wound care management plans based on evidence-based practices.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Provide education to patients and families regarding wound management and prevention.
- Stay current with advancements in wound care management and participate in continuous professional development.
- Perform point-of-care testing and interpret results.
- Maintain accurate and up-to-date patient records using electronic health systems.
- Adhere to all applicable federal, state, and local laws and standards related to patient care and documentation.
- Travel daily to patient homes, assisted living facilities, and similar environments.
Qualifications:
- Active Nurse Practitioner license in the State of South Carolina.
- No sanctions or disciplinary actions on current or past licenses.
- National board certification (AANP or ANCC) in family, adult-gerontology, or related specialty.
- Wound Care Certification is preferred
- Current BLS certification.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Ability to work independently in non-traditional healthcare settings.
- Strong clinical, communication, and organizational skills.
Preferred Qualifications:
- 1 years of clinical experience as a Nurse Practitioner, particularly in home health, geriatrics, wound care or community-based care.
- Familiarity with EMR systems and mobile documentation tools.
Work Environment & Travel:
- Must be able to travel 30–45 miles from your home base to reach patients.
- Mileage reimbursement provided.
