Overview
We are a Non-Medical In-Home care agency. This position requires one to make In-home-visit assessments on new VA Clients. Travel around the Greater Jackson area is required.
We are seeking a dynamic and dedicated Part-time Assessment Manager to join our multidisciplinary team. In this pivotal role, you will oversee comprehensive client assessments, coordinate evaluations, and support individualized care planning for diverse populations. Your expertise will ensure timely & accurate assessments, effective services planning, and seamless integration of services across the disabled Veteran sectors. Responsibilities
- Conduct thorough intake assessments and evaluations on new clients utilizing in-home interviews with the client.
- Manage documentation accurately ensuring compliance with HIPAA regulations and administrative code standards.
- Perform crisis management and crisis intervention if necessary, ensuring client safety and appropriate escalation procedures are followed.
- Responsibilities may increase as needed.
- All time is flexible.
Requirements
- Must be dependable! and able to meet the requirements and responsibilities of the job.
- Proven experience working with vulnerable adults, developmental &/or physical disabilities, and dementia.
- RN strongly preferred but will consider others who demonstrate supervisory experience leading healthcare teams within social work settings.
- Excellent communication skills for client interaction, family engagement, and multidisciplinary team collaboration.
- Ability to manage multiple priorities efficiently while maintaining attention to detail in medical records and case documentation.
- Working knowledge of HIPAA regulations, and cultural competency practices when working with diverse populations.
