Responsibilities
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or wheelchairs.
- Monitor and observe patients’ conditions, including vital signs and overall well-being, documenting changes accurately in electronic health record (EHR) or electronic medical record (EMR) systems.
- Administer medications and treatments following medical documentation protocols and under supervision when required.
- Support clients with specialized care needs such as Alzheimer’s disease, dementia, memory care, hospice support, or caring for individuals with developmental disabilities.
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment aligned with HIPAA privacy standards.
- Maintain clear communication with healthcare teams regarding patient progress and medical records while respecting confidentiality.
- Use medical terminology appropriately during documentation and patient interactions to ensure precise communication.
Requirements
- Proven experience in home health care or long-term care settings such as nursing homes or assisted living facilities.
- Strong skills in patient observation, medication administration, catheter care, hospice support, and dementia or Alzheimer’s care.
- Knowledge of EMR/EHR systems for accurate medical documentation and record keeping.
- Valid CPR certification and familiarity with medical terminology are required; additional certifications such as HIPAA training are preferred.
- Ability to work independently while managing multiple client needs in a fast-paced environment.
- Experience working with individuals with disabilities or developmental challenges is highly valued.
- Physical ability to assist with mobility support using equipment like Hoyer lifts or transfer belts; capable of standing, walking, lifting, and performing light cleaning tasks.
