- Provide direct nursing care to senior residents in skilled nursing, rehab, assisted living, and memory care settings.
- Administer medications and treatments per physician orders and document accurately in the EMR.
- Assess, monitor, and report changes in resident condition; collaborate with interdisciplinary team.
- Develop, implement, and update individualized care plans focused on quality outcomes and dignity.
- Educate residents and families on care plans, treatments, and health maintenance.
- Supervise and support LPNs, CNAs, and other care team members to ensure person-centered care.
- Maintain compliance with infection control, safety, and regulatory standards.
- Participate in care conferences, quality initiatives, and faith-based mission activities.
