Job Title: Charge Nurse / Nurse Supervisor
Schedule: PMs 72-80 hours over a two-week period
Reports To: Director of Nursing (DON)
Job Summary
The Charge Nurse is responsible for coordinating resident care through assessments, quality assurance initiatives, and communication with physicians and licensed staff. This role provides leadership and support to nursing staff under the direction of the Director of Nursing (DON), ensuring high-quality, resident-centered care.
Key Responsibilities
Resident Care Coordination
- Review shift reports and communication from the previous 24 hours; identify and follow up on areas of concern
- Assess and respond to acute changes in resident condition; notify physician and initiate follow-up assessments
- Perform lab draws, monitor results, and ensure proper documentation and follow-up
- Communicate physician orders and updates to appropriate staff and ensure implementation
- Notify residents and families of medication changes, condition updates, and physician communications
- Assist with physician/practitioner rounds and transcribe orders accurately
Admissions & Ongoing Assessments
- Assist with new admissions and onboarding processes
- Complete and document:
- Skin assessments
- Physician and discharge orders
- Medication and treatment records
- Pain assessments
- Side rail/device assessments
- Initiate and update care plans
- Maintain accurate resident care documentation in electronic charting systems (ECS)
- Prepare appointment packets and manage scheduling needs
Clinical Duties
- Provide IV therapy and assist with treatments
- Perform Accu-Checks and administer insulin as needed
- Assign and follow up on abnormal vital signs and weight changes
- Support resident education and health teaching needs
Quality Assurance & Compliance
- Assist DON with staff audits and quality assurance projects
- Complete incident reports per policy and initiate investigations
- Ensure compliance with documentation standards and care protocols
- Assist with weekly summary and Medicare charting
Staff Leadership & Support
- Supervise daily assignments and ensure completion of resident care tasks
- Address and resolve staff concerns affecting the work environment
- Coordinate staffing coverage, including replacing call-ins
- Provide guidance and support to nursing staff
Additional Duties
- Attend care conferences as scheduled
- Review calendars for appointments, conferences, and events
- Maintain accurate resident care cards and records
- Perform other duties as assigned by nursing leadership
- Must be currently licensed to practice as a Registered Nurse in the state of Wisconsin.
- Or has graduated from an accredited program, carries a temporary permit to function as a Graduate Nurse in the State of Wisconsin and is awaiting State Boards
- Strong clinical assessment and critical thinking skills
- Experience in long-term care or skilled nursing preferred
- Excellent communication and leadership abilities
- Proficiency with electronic charting systems
