Bethany Health Care is looking for a Director of Social Services for a Skilled Nursing Facility.
Part Time 32 hours per week
JOB SUMMARY
Assists in meeting the psychosocial needs of residents in a long-term care facility. Collaborates with the inter-disciplinary team to develop and carry out individualized care plans to enable each resident to achieve their optimal level of health and well-being and maximize their quality of life. QUALIFICTIONS
MA LCSW required.
A minimum of 2-3 years' experience in a long-term care setting.
Solid working knowledge of discharge planning, Medicare/Medicaid, OBRA regulations, community resources, social work methods and techniques.
Excellent communication, interpersonal, and computer skills required.
Knowledge of electronic medical records, MDS, and Care Plans.
