Job Purpose:
As an RN or LPN, the Patient Navigator serves our patients and families with care and respect while committing to advocate and better the lives of those we serve. Under limited direction, this position functions as part of the multidisciplinary team to coordinate patient care throughout the continuum of care and serves as a clinical resource for care management. As a Nurse (RN/LPN), the Patient Navigator is responsible for coordinating and facilitating patient needs including, but not limited to, internal and external patient transports, scheduling, and internal specialist rounds. The Patient Navigator will remain flexible and responsive when changes occur in patient activity and workload to support team while maintaining a strong sense of patient and family focus. |
Work Schedule:
Full-time, Monday - Friday, usually 8:00a - 4:30p, with flexibility as needed for early or late external patient transports (with notice).

Responsible for coordinating the health care needs of the patient. Connects patients and families to resources and support systems, assists with interactions between care providers, schedules appointments, streamlines paperwork and acts a patient advocate. |

Performs safe and efficient transferring and escorting of patients to and from designated areas. May require patient lifting, pushing and/or pulling. |

Cultivates and supports primary care and specialty provider co-management with timely communication, inquiry, follow-up and integration of information into the care plan regarding transitions in care and referrals. Communicates information across the care team. |

Serve as the contact point, advocate and informational resource for patient families, care teams, and specialists. |

Qualifications:
Education: |
Completion of diploma Nursing Program (LPN or RN) |
Experience: |
Minimum of (2) Years of experience as RN or LPN is required. Pediatric experience is preferred. |
License/Certification/Credential: |
RN/LPN: Current LPN or RN license in state of Oklahoma. CPR certification within 1 year of employment. Valid driver’s license. Current in check off for med pass and sedation AND Current valid Basic Life Support (BLS) certification and Pediatric Advanced Life Support (PALS) certification (or ability to obtain within 1 year of hire). Valid driver’s license. |
